Dr. Olson has assisted pilots from around the world with aeromedical certification.  Pilots with complex medical conditions and who may be in need of consultation or have questions regarding their condition in relation to FAA medical certification can schedule a phone consultation below.  Phone Consultation Calls are either 30 or 60 minutes depending on the type of case.

The phone consultation provides the opportunity to discuss the details of your case, to answer questions you may have, as well as determine possible pathways forward for your case.

Step 1 - Book the Appointment Below

​​Step 2 - Phone Consultation Questionnaire and Airman Medical History Forms:

In preparation for the call, please fill out the Phone Consultation Questionnaire and the Airman Medical History forms (downloadable below) and submit to Dr. Olson by either emailing to doc@flight-med.com or faxing to 210-640-1938 at least 24 hours in advance of your scheduled call.

Step 3 - The Phone Consultation

Dr. Olson will call you at the date & time of your appointment.

​

If your case involves both substance related and mental health diagnoses, choose the Drug & Alcohol option. 

​

If your case involves ADHD along with other mental health conditions, choose the Mental Heath - Complex or ON Medications option.

​

Keep in mind, there is a lot to discuss during these calls.  If a shorter duration call is scheduled, there may not be enough time for the call to be worthwhile.  In that case, a subsequent call may be needed.

Airman Responsibilities: 

You agree to provide accurate and complete information during your Phone Consultation Call. No records or documents are required prior to the call. Any guidance or recommendations provided by Flight Med are based on the information you disclose during the consultation. If additional records or documentation are needed to provide further assistance, Flight Med will advise you accordingly

 

Your Medical History and Records:

Please be prepared by knowing and understanding your diagnoses, medical treatments and medications, and current treatment plan as directed by your treating provider.  It may be beneficial for you to have documents available for your review during the call.  This may include medical documents (visit notes, test results, operative reports, imaging study reports), legal documents, court documents, and/or police documents.

The phone consultation service does not include document review.  Please DO NOT send legal, medical, or other documents prior to or after the consultation call unless specifically discussed and requested by Dr. Olson.

 

Continued Consultation:

The phone consultation service does not include or imply continued consultation services by Dr. Olson as a HIMS AME or consulting AME beyond the duration of the consultation phone call, and for HIMS cases the call DOES NOT constitute a HIMS AME assessment.  Not every case requires ongoing consultation beyond the consultation call, however before taking on any case as a HIMS AME or consulting AME, the first step is a consultation phone call.  Dr. Olson can accept any phone consultation appointments, however he may or may not be able to accept new cases for further consultation as a HIMS AME or consulting AME based on his workload.  

 

Basic Med:
